ADMISSION PROCEDURES
Each student must first confirm participation to the clinical
rotation in Family Medicine by forwarding an email to the

turn in clinical training logs/patients logs (signed by
the physician) and a completed evaluation by the
student on the preceptor performance.
These forms are part of the registration package and
are ready to download on the site. Completed forms
