Home & Hospital for the Aged and the Center for Haitian Studies, Health & Human Services - GMHETC
ADMISSION PROCEDURES
Each student must first confirm participation to the clinical rotation
in Family Medicine by sending an email to the following address:

  • Submit the required documentation before orientation date
  • Attend orientation

END OF EACH CYCLE REQUIREMENTS:
At the end of each two-week cycle, each student  will
turn in clinical training logs/patients logs (signed by
the physician) and a completed evaluation by the
student on the preceptor performance.

These forms are part of the registration package
and are ready to download on the site.

SITE PROCEDURES, SCHEDULE & EVALUATION
Please report to your assigned site at 8:30am or (as
indicated on schedule
) for weeks 1 & 2.  Then,
communicate with the site contact as to the daily
schedule for the physician.

Please note that each student will provide the
preceptor at each site with
a blank student evaluation
form bearing the student's photo, name and
information.

Please click above and print student evaluation form
and copy your photo on it as indicated in sample
page above before giving it to preceptor.

Please note that completed student evaluations by
preceptors will be collected by the CHS staff.
By
policy and under no circumstances, should any
student collect completed evaluation from
preceptors.  Even in cases where a preceptor
would, by mistake, hand a student his/her
evaluation. The student should refuse and
should immediately alert

  1. Drug Screen 12 Panel (no more than a year old)          
  2. Police / Criminal Background (Original Hometown or
    National)
  3. Physical Examination (no more than a year old)
  4. Immunization Form (Titers, MMR, Varicella, Hep B, PPD)
  5. Flu Vaccination
  6. Demographic Forms 1 & 2 as provided in package
  7. Evaluation form with photo attached (please bring two
    passport size photos).

Please note that the topic of the research paper is
to be chosen from a pool of pre-approved subjects.

Other Notice:

Scheduling and Assignment to
Training Sites:
Students are assigned to specific training sites
based on a number of criteria.  Scheduling of
rotations and assignments of students to
training sites are final and will not be changed
to accommodate student preferences.  
Students are responsible for arranging their
own transportation to ensure timely arrival at
rotation sites. We cannot take carpooling
arrangements or other student preferences
into consideration when making assignment
decisions.


Appeal of Final Grade:
Final grades are computed, by our staff, based
on the evaluation forms completed and
reported to us by preceptors. These grades
are transferred directly to the Clinical Dean
and/or staff designated by your Medical
School.  Any dispute about the final grade must
follow the medical school’s procedure for
appealing grades.  Under no circumstances will
a final grade be changed by our staff outside
of the medical school’s appeal procedure.  If
students have concerns during the rotation
about how they will be evaluated, these
concerns should be brought to the attention of
our rotation coordinator who will resolve
difficulties with preceptors prior to grades being
issued.
