Scheduling and Assignment to Training Sites

Students are assigned to specific training sites based on a number
of criteria.  Schedule and assignment to training sites are final and
will not be changed to accommodate student preferences.  

Students are responsible for arranging their own transportation to
ensure timely arrival at rotation sites. Carpooling arrangements and
other preferences have no weight in site assignment.
Site Procedures, Schedule & Evaluations

Please report to your assigned site at 8:30am or (as indicated on
schedule
).  Then, communicate with the site contact as to the
daily schedule for the physician.

Please note that each student will provide the preceptor at each
site with
a blank student evaluation form bearing the student's
photo, name and information.

Please click above and print student evaluation form and copy your
photo before giving it to preceptor. Please note that evaluation
forms are site-specific.

Please note that staff will collect evaluations directly  be collected
by staff.
By policy and under no circumstances, should any student
collect completed evaluation from preceptors.  Even in cases
where a preceptor would, by mistake, hand a student his/her
evaluation. The student should refuse and should immediately
alert

Research Paper

Please note that the topic of the research paper is to be chosen
from a pool of pre-approved subjects. Click below to download

RESEARCH PAPER PRE-APPROVED TOPICS

Criteria for the format of research paper:
Four (4) pages long minimum
Double-spaced
12 pt font
Please include sources page or bibliography
Hard copy
Hand delivered
Due date:
Friday of week 4 of rotation

End of Roration Requirements

At the end of each two-week cycle, each student  will turn in
clinical training logs/patients logs (signed by the physician) and a
completed evaluation by the student on the preceptor
performance.

These forms are part of the registration package and are ready to
download on the site.

Final Grade

Evaluations are sent directly to the Clinical Dean and/or staff of
Medical School.  The Medical School handles all grades dispute.
